Algorithm for Managing Patients with Pain Syndrome

ABSTRACT

INTRODUCTION. Pain syndrome in the back and large joints is the most frequent complaint at outpatient appointments. Osteoarthritis is one of the most common chronic musculoskeletal diseases worldwide, being the leading cause of chronic pain and disability. One of the possible causes of the development of musculoskeletal low back pain is degenerative changes of the intervertebral disc and osteoarthritis of the facet joints. The pathogenesis of facet syndrome is based on the imbalance between anabolic and catabolic processes within hyaline cartilage and subchondral bone. Therapy of patients with diseases of the musculoskeletal system should be carried out in a complex and include at all stages: diet, weight control, optimal motor regime, long-term use of SYSADOA (Symptomatic Slow Acting Drugs for OsteoArtritis) and pharmaconutraceuticals. ln some cases, the use of orthopaedic devices (orthoses, insoles) is indicated.

CONCLUSION. An integrated approach allows improving the quality of life and delaying the need for surgical interventions and the need for endoprosthesis. Of the SYSADOA available on the Russian market, chondroitin sulphate (Chondroguard®) has great evidence. lt is also promising to further study the efficacy of a new pharmaconutraceutical Chondroguard® TRlO, which was developed taking into account scientifically substantiated studies on the use of chondroitin sulfate, glucosamine sulfate and non-denatured type ll collagen and. This is optimal for nutritional support of patients with diseases of the musculoskeletal system. The presented algorithm allows optimising the diagnostic and therapeutic algorithm for patients with joint and back pain.
